Payment for Insurance Visits
I am in-network with Aetna, United, and Tricare, and the coverage must be your primary insurance in order for your visit to be covered. You are responsible for contacting your insurance and confirming your eligibility and benefits. Both you and your baby are considered by clients, so both must be insured in order to receive full insurance coverage. If either you or your baby are out of network, the Client may be responsible for a portion of the visit up to but not exceeding the full fee (detailed below).
AS OF SEPTEMBER 1, 2026 - United Healthcare may no longer be covering infant lactation visits. What this means for your visit is that some of your visit cost may not be covered by insurance and you may be responsible for a portion of the visit up to but not exceeding the full fee (detailed below). You may also bre required to request a referral from your primary care physician or pediatrician to receive lactation insurance benefits under my care.
A credit card will be collected at the time of booking to reserve the appointment, and you will be responsible for the full fee in the event that insurance refuses to cover the visit. Both parent and baby must be in-network, otherwise the Client may be responsible for a portion of the visit up to but not exceeding the full fee (detailed below).
Your insurance may ask for my NPI (1588300164), business name (Brook Farm Lactation and Doula), and practitioner name (Alexandra White), to confirm your benefits.
Initial visits are $350 and last typically 90-120 minutes. Follow up visits are billed at $175 an hour.
All home visits with a distance longer than 20 miles from zipcode 10506 (Bedford, NY) will be charged a $50 travel fee that is not covered by insurance.
If you need to cancel or reschedule your appointment, a 24-hour notice is appreciated. Any appointment cancelled within 24 hours without rescheduling will be charged a 50% ($175) fee.
